Complete the following forms, (1) ETA 855 - Request for Determination of Entitlement to TRA, and (2) UCB 345 - Acknowledgement of TRA Eligibility Deadlines and return to the Trade Program Unit via fax 850-921-3427, or mail to: Florida Department of Economic Opportunity, Trade Program Unit, 107 E. Madison Street, Tallahassee, Florida 32399-4120.


Print all forms listed and keep a copy for your records. Call the local TAA Coordinator within your area for an appointment to discuss your potential benefit options
